Leukemias and Metabolic Syndrome: Clinical Features, Bidirectional Relationship

Abstract
Acute leukemias represent a group of malignant hematopoietic disorders characterized by the uncontrolled proliferation of abnormal cells. Within the overall incidence of hematologic malignancies, acute leukemias (both myeloid and lymphoblastic) constitute a significant proportion, occurring at an estimated frequency of approximately 4 cases per 100,000 adults for acute myeloid leukemia and 1.5 cases for acute lymphoblastic leukemia. Key indicators of treatment efficacy include complete remission and long-term survival.
Modern therapeutic strategies, encompassing polychemotherapy and hematopoietic stem cell transplantation (HSCT), have led to substantial improvements in the overall survival rates of patients with acute leukemia. However, the intensity of treatment is often accompanied by the development of various complications, among which metabolic disturbances are of particular clinical relevance. These alterations may present as both acute events (such as hyperglycemia and electrolyte imbalances) and delayed effects (including lipid metabolism disorders, osteopenic processes, and disruptions in endocrine regulation).
While the toxicity of antineoplastic therapy is widely discussed in the current literature, the specific features and dynamics of metabolic disturbances in adult patients with acute leukemia undergoing pathogenetic treatment (including HSCT) remain insufficiently studied. The absence of standardized diagnostic and monitoring algorithms for metabolic abnormalities at different stages of treatment complicates timely intervention and the prevention of severe complications.
Thus, the relevance of this topic is underscored by the need for systematic verification of metabolic disturbances throughout the disease course – from initial diagnosis to late post-treatment periods – which could enhance diagnostic and therapeutic processes, improve patient quality of life, and ultimately contribute to better prognostic outcomes.
