A Study the Prevalence of Vertebral Fractures in Patients with Hodgkin’s Lymphoma
and its Association with Bone Mineral Density Assessed by Dual-Energy X-Ray Absorptiometry

Abstract
Background: Phpatients with Hodgkin lymphoma are young, socially active individuals with a long life expectancy. However, the occurrence of late treatment complications, such as osteoporosis, is a concern.
Objective: to determine the prevalence of vertebral fractures in patients with Hodgkin lymphoma and determine its association with osteoporosis.
Material and methods: CT, PET/CT, and densitometry results were analyzed in patients with Hodgkin lymphoma and a group of healthy controls. Inclusion criteria for the study group were: diagnosis of Hodgkin lymphoma; therapy including glucocorticosteroids; and a CT and/or PET/CT series over at least 1 year. Exclusion criteria: specific bone lesions.
Result: vertebral fractures were detected in 47% of women and 41% of men. Fractures of two or more vertebrae were the most common (62% and 67%, respectively). The average number of vertebral fractures per patient was 2.2. Genant grade 1 fractures were the most common (59%). In 27% of cases, the severity of vertebral fractures increased during the observation period. In the control group, vertebral fractures were detected in 15% of cases. 80% of patients had single vertebral fractures, and 20% had double vertebral fractures. All fractures were Genant grade 1. Of 14 men and 22 women with Hodgkin lymphoma and vertebral fractures, only 1 man (7%) and 4 women (18%) showed a decrease in BMD on DXA.
Conclusions: a significant prevalence of vertebral fractures was found in patients with Hodgkin lymphoma. The prevalence of vertebral fractures increases sharply from disease onset to and during remission. BMD in patients with Hodgkin lymphoma, measured by DXA, has a weak relationship with the prevalence of vertebral fractures in this patient group.
